A Committee is going to review Jayden Conner’s case
The day before the 28 day review, Dr Zim prepared a confidential assessment into an encrypted server, accessible only to his private network. Years later, he passed it to Marcus to pass on to me, part of the history lesson I suppose? Or showing off his power? Probably both.

‘..subject exhibits profound regression.’ he wrote, the tone clinical yet laced with satisfaction. ‘Initial resistance to nomenclature eroded via operant conditioning; zero utterances of prior delusion alias ‘Jacob Ellis’ in 14 days. Physical alterations integrate seamlessly, his nose, anus and nipples and ears transformed, facial indicators (lips and jaw line) altered, scars cicatrized, tattoos unblemished, septum and nipple piercings healed without infection. Dental work complete.’
‘..Subject’s anal modifications healed fully by day 3. This has allowed for twice daily dilations by nursing staff. Initially with a 3cm diameter silicone vibrator, this has moved to 6cm diameter, twice daily, reflecting the larger penis sizes the subject will have serviced during his active sex work, and which he will be expected to service in future. In due course, the subject will practice on himself, albeit under close supervision. This will re-acquaint the subject with anal use, including hygiene protocols, lubrication and technique.’

Years later I saw the minutes, part of the history of becoming Jayden Conner, the story of my colonisation. Jacob didn’t stand a chance here did he?
Three independent psychiatrists, one taking on the role of Chair, a social worker and a rep from the NHS oversight board were now in session as the Review Committee. Dr. Zim opened, 'our subject today, Jayden Conner, has shown marked stabilisation over the past four weeks. Initial severe agitation upon admission, delusional episodes centred on an alternate identity, have subsided under a standard medication regime. Behavioural conditioning via nicotine reinforcement has yielded 85% compliance with identity protocols; verbal affirmations of 'Jayden' now dominate interactions, with no spontaneous regressions noted in the last fortnight.' The files show a photo: a figure slumped in a too-tight tracksuit, cigarette tucked behind each ear, eyes glassy from the morning dose of medication.
The social worker asks, ‘Should we be encouraging smoking, as an incentive method?’
‘The patient already presents with an established nicotine habit of circa 20 a day. We judge withdrawal overly stressful in the circumstances, and the habit itself, supervised, has proved a helpful compliance anchor.’ The minutes record approval, satisfied with the clinical view. The social worker pursues a different line of questioning. ‘I fully understand the radical case for body modifications, and the progressive agenda, (general assent by the Committee here is noted). However, I observe from the report some considerable resource is put into Mr Conner’s re-introduction to sex work, in particular the daily dilation protocols. Could Dr Zim comment on that please?’
Zim was only too happy to comment. ‘Jayden Conner’s records show a long history of sex work, all as the receptive partner, colloquially, Mr Conner is what is known as a ‘submissive.’ The intention is to override the Jacob delusion’s insistence on middle class status and identity markers with thorough immersion in the gritty realities of sex work. The dilation protocol is an important first step. I note this also includes Mr Conner receiving the uncleaned instrument orally, to ensure realism and erosion of the delusional identity’s insistence on unrealistic and, quite frankly, bourgeois and reactionary hygiene standards. He is, after all, returning to his authentic identity, which is, I emphasise to the Committee, the evidence-based actual identity.
The social worker wasn’t finished. ‘In the addendum the report mentions body fluid ingestion and reacquainting him at a fundamental level? Could you elaborate please?’
‘Yes, urine is collected in the clinic environment and then treated. The patient is required to ingest 500ml daily, split between two sessions. He is obliged to express genuine gratitude and pleasure. We insist on this, it is a vital part of the therapy. In addition, we have canvassed male volunteers for sperm donation, and have sufficient for Mr Conner to ingest 10ml daily. This is taken in a single session. He is required to hold this volume in his open mouth for a period of five minutes, to fully condition him to the taste, texture and sensation. He is then required to thank the staff, smile, and then swallow to completion. This all replicates, as best we can in a clinical setting, what the evidence tells us are the actual circumstances of his sex work. Veracity and authenticity here is key, and we believe we have achieved that, although we would welcome any further advice from the Committee on this approach. It is important to note that this regular exposure to male body fluids, will further re-condition Mr Conner as a full time sex worker.’
Zim steered the discussion in a different direction. ‘One external factor merits mention: you may well have heard the publicised disappearance of Jacob Ellis, the young City of London banker. It's pretty clear there is some connection between our patient Jayden Conner and Jacob Ellis. Quite what that is, we don’t know. All we can say for sure is this individual is the focus of his delusion. However, notwithstanding the obvious risks, I propose we are on sufficiently secure ground to allow an interview with the police, to see what Jayden knows about Jacob. This may help the police with their enquiries; it might also reveal information relevant to our own case, if the Committee is content?'
The minutes record a brief consultation, then there is a general agreement with a few questions on risk assessment: ‘a supervised interview with the police, no more than 30 minutes, with staff present via CCTV. Extend patient hold to 90 days pending outcomes of a day release into the community.'
